Primary PartnerCare is a physician-driven organization (MSO, MSSP ACO, Medical Group & IPA). We were founded on the belief that private practice primary care doctors in partnership with their patients produce optimal health. Established in 2012 to help independent physicians meet the demands of healthcare reform, we provide tools and resources for doctors to remain independent and thrive. Effective January 2014, one of our independent physician associations, Primary PartnerCare Associates IPA, was selected by CMS to participate in the Medicare Shared Saving Program. As participants, our doctors are able to collaborate and provide additional care coordination and support programs for their patients. Equally important, our doctors are committed to measuring outcomes of the care we provide, and assuring that our patients receive their preventive care services. With primary care and specialty physicians dedicated to seeking excellence in patient care, technology, and innovation, Primary PartnerCare® strives to achieve a healthier and happier future for both our doctors and their patients.
